It will be very helpful for me to know if your interest is for individual use or as part of a group. Please answer the following questions to the extent you feel comfortable before submitting the request for either the Book Club Discussion Questions or the Support Group Discussion Questions (both in pdf format). Answering these questions will help me better understand the audience for providing future questions, wording changes, or possibly future information.

Your answers will be appreciated and will be kept confidential; however, you do not have to answer any of the questions to receive the Discussion Questions documents.
